Rabbit pus is caseous or "cheesy"

This little rescued bunny arrived in our practice with a swollen, closed eye. She lived with other pets, and her owner couldn't say what had happened while they were alone. In her first visit she let us clean and oper her eye, check that there was no foreign body within, that anatomically it was intact. She was prescribed some antibiotics and eyedrops, after a fluorescein test that ruled out any corneal ulcer. In an oral examination with an otoscope, for a preliminary exam, it was found that incisors were being clipped at home, and molars could contact with each other.

Este pequeño conejito, adoptado, llegó a la clínica con un ojo cerrado e hinchado. Vivía con otras mascotas, y la dueña no sabía qué había pasado mientras estaban solos. En su primera visita nos dejó limpiarle y abrirle el ojo, comprobar que no había un cuerpo extraño dentro, y que anatómicamente estaba intacto. Se la recetó antibióticos y un colirio tras descartar úlceras corneales con un test de fluoresceína. En un examen bucal con otoscopio, como examen preliminar, se encontró que los incisivos se habían cortado en casa, y que las muelas contactaban bien entre ellas.

Two days later her owner noticed she was discharging some white fluid from the affected eye, that was, again, closed, and impossible to treat with eyedrops. At this moment the alarm went off, suspecting of a bigger problem. Epiphora and dacryocystitis are sympthoms of a more serious condition, dental problems. Under sedation it was clear that the infection came out through the lacrimal duct. After half an hour doing our best to flush the tear gland, we gave up.

Dos días después, la dueña notó que salía un líquido blanco del ojo afectado, que estaba nuevamente cerrado, y era imposible de tratar con el colirio. En este momento saltaron todas las alarmas, sospechando un mayor problema. Epífora y dacriocistitis son síntomas de algo más seri, un problema dental. Bajo sedación, se hizo evidente que la infección venía del conducto lacrimal. Tras media hora intentando todo por irrigarlo, tuvimos que rendirnos.

Several Xrays were performed, producing a very severe dental problem. We already knew of her incisors malocclusion (and that clipping them in house could have exacerbated the problem). Now, it was clear that dorsal limits of the imaginary line between the nasal bone-occipital protuberance had been passed, as well as  alteration in the dental occlusal plane (end palate- 1/3 tympanic bulla ).

Se llevaron a cabo varias radiografías, viéndose que había un grave problema dental. Ya sabíamos de la maloclusión de los incisivos (y el cortarlos en casa pudo exarcerbar el problema). Ahora, estaba claro que el límite de la línea imaginaria entre el hueso nasal y la protuberancia occipital se había traspasado, así como alteraciones del plano oclusal (fin del paladar a 1/3 de la bulla timpánica).

On another radiography, we could notice some reduce bone density* and thinning of mandibular ventral border, root elongation and divergence,  a very clear alteration in dental occlusal plane, splaying of dental apices and crowns, and abscess formation.

Once disease is noted, changes are normally irreversible. Teeth removal is not always curative either, for only the removal of the germinal tissue will prevent tooth grow and, unfortunately, teeth are growing during all the bunny's life.

En otra radiografía pudimos ver una reducción de la densidad ósea*, y adelgazamiento del borde ventral de la mandíbula, alargamiento y divergencia de las raíces, una clara alteración del plano oclusal, separación de los ápices y coronas dentales, y formación de un absceso.
Una vez la enfermedad es diagnosticada, los cambios suelen ser ya irreversibles. La extracción dental no resuelve el problema en todos los casos, ya que hace falta extraer también el tejido germinativo que evite que el diente vuelva a nacer y, por desgracia, los dientes están creciendo durante toda la vida de los conejos.

*Pet rabbits are usually fed an excess of calcium that rules out any mineral deficiency. However, other metabolic conditions, such as hyperparathyroidism could be underneath.
*Los conejos caseros suelen tener una dieta sobrada en calcio que descarta déficit de minerales. Sin embargo, otros problemas metabólicos, como hiperparatiroidismo, podrían ser otra causa.
