Heart failure/fallo cardíaco

Tess is a neutered female, 9ish y.o. rescue dog who, suddenly, was weaker, loosing weight and fainting. Her owner, one of the vet nurses from our team, took herself some blood samples. We could not find any heart murmur on auscultation. Results from the lab were inconclusive. T4 slightly low, didn´t match with Tess symptoms.

Tess heart size and sounds
were unremarkable

Her glucose levels in blood, although low in first test, they were also within normal range in further tests. Her hematology was fantastic, ruling out anaemia or infection. Her biochemistry was driving crazy both the vets within Companion Care and those from the idexx lab. 

Sus niveles de azúcar en sangre, aunque bajos en el primer test, resultaron ser normales. Su hematología estaba estupenda, descartando anemia o infección. Su bioquímica nos estaba volviendo locos a los veterinarios e Companion Care y a los del laboratorio de idexx.

We decided to run some radiographies on torax and abdomen. She was not coughing, getting more tired, ascitis or any of the typical symptoms of heart failure, but we needed to find out what was wrong on her. Her abdominal Xray was unremarkable, whereas her chest Xray dispayed a bigger heart than expected.

Decidimos hacer unas radiografías en tórax y abdomen. No tosía, ni se cansaba más, ni había ascitis, ni tenía los síntomas típicos de un fallo cardíaco, pero necesitábamos averiguar qué le pasaba. La radiografía del abdomen no condujo a nada, pero la del pecho mostró un corazón más grande de lo esperado.

Finally, it was decided to run a cardiac proBNP test. We noticed that Tess serum kept clotting, even after centrifugation and breaking it, which was an issue to run the test. Anyway, the sample was sent to idexx.

That same evening Tess took some ACE inhibitor tablets, which made her sick, to her owner's dismay. 

Finalmente se decidió hacerle un test cardíaco proBNP. Nos encontramos con que el suero sanguíneo se coagulaba, incluso tras ser centrifugado y roto, lo que era un problema para hacer el test. Aún así, se envió la muestra a idexx.

Esa misma noche Tess empezó con IECAs, lo que la hicieron vomitar, para desesperación de su dueña.

The turning point happened once the proBNP results arrived. So far they were the highest that idexx had ever got. We had to put Tess on a urgent heart treatment, whatever side effects the medication causes. Fortunately, it was not as bad as we thought, and now Tess is not collapsing anymore, gaining weight and being herself once more :)

El punto de inflexión llegó con el resultado de pro BNP. Por ahora, ha sido el más elevado que idexx ha registrado. Teníamos que poner a Tess en tratamiento cardíaco, a pesar de los efectos secundarios. Por suerte no fue tan mal como pensábamos, y ahora Tess ha dejado de desmayarse, ha ganado peso y vuelve a ser ella una vez más :)
