Septic arthritis in a senior dog

On a freezing winter evening we got the visit of a senior female dog who had had a lump on her right hock for about 7 years. Up until now it did not give her any problem and never was investigated.

Al anochecer de un frío dia de invierno recibimos la visita de una perrita mayor con un bulto en su tobillo derecho desde hace 7 años. Hasta entonces no le había dado problemas ni nunca se examinó.

However, within the past 3-4 weeks she had gradually become more uncomfortable, cannot weight bear and, on initial presentation, there was no obvious heat or swelling around her hock, but a very marked bone enlargement which was not painful on palpation.

Sin embargo, en las últimas 3-4 semanas la ha estado molestando cada vez más, no se puede apoyar en el tobillo, y en la consulta no estaba caliente ni hinchado, pero sí un muy marcado cayo óseo que no le dolia en la palpación.

Her owner had been giving her some joint supplements, and these seemed to help at fist, but not in the past 4 weeks. Ideally we would need to sedate her and perform some radiographs in both hind legs and hips, but being an elderly dog her owner is very concern any sedation may do more harm than good. We decide to give her a go with some pain relief for few days and recheck her condition.

Su dueña le había dado un suplemento para las articulaciones, y parecía ayudar en un principio, pero no durante las últimas 4 semanas. Lo aconsejable sería sedarla y hacerla algunas radiografías en ambas patas traseras y la cadera, pero al ser tan mayor, la dueña está preocupada de que la sedación traiga más daño que beneficio. Decidimos darle una oportunidad por unos días con un analgésico y revisar la evolución.

In three days time her condition worsen to the point that now the hock is markedly swollen, oedematous, painful and hot. 

En tres días el problema empeoró hasta el punto que el tobillo está marcadamente hinchado, edematoso, doloroso y caliente.

After a preanaesthetic blood test, and a mild sedation, some radiographies were taken, showing very irregular hock bones, confirming a severe arthritis and ruling out any fracture or bone tumour. Accidentally it is found an old cruciate ligament rupture on her left hind leg as well.

Tras unas analíticas sanguíneas para preanestesia, y una sedación suave, tomamos algunas radiografías, confirmando una artrosis severa, y descartando fracturas o tumores óseos. Casualmente también encontramos una antigua rotura de ligamento cruzado en la pata trasera izquierda.

A needle aspiration exposes some purulent fluid, and a sample was sent to an external laboratory for further investigation.

Una muestra por aspiración con aguja extrae un líquido purulento, y parte se envía a un laboratorio externo para su análisis.

Her treatment consisted on aggressive intravenous antibiotherapy (cephalexine and metronidazole), strong pain relief (buprenorphine), and cold packs locally, several times a day. She stood hospitalized for 5 days, continuing her treatment orally at home. 

Su tratamiento consistió en una agresiva antibioterapia intravenosa (cefalexina y metronidazol), un potente analgésico (buprenorfina), y aplicar frío localmente varias veces al día. Permaneció hospitalizada durante 5 días, y siguió con su tratamiento vía oral en casa.
